The purpose of this study was to compare the electrocardiographic diagnosis of atrial fibrillation (AF) made from the 12-lead ECG (12-LEG) with that made from a lead (modified limb lead [MLL]) taken from a patient's hand. Ninety-six patients (33 with AF) were monitored over a 2-year period. The mean duration of AF was 15.3 hours (range, 0.5-48). Fifty-five patients had atrial fibrillation and 30 patients had atrial flutter. Each patient had an 12-LEG and MLL simultaneously recorded. The rhythm strip obtained from the MLL was divided into 2 groups, similar to the 12-LEG rhythm strips. The first was composed of all 12 MLL leads plus the 12 ST and V5 leads, and the second was composed of all ST and V5 leads alone. The recordings were then reviewed by 2 blinded observers. The overall results for the MLL lead, 12-LEG, and "MLL + V5 leads" were 45%, 40%, and 73%, respectively, for the diagnoses of AF. The sensitivities and specificities for each rhythm were as follows: MLL, 78% and 90%; 12-LEG, 45% and 93%; MLL + V5 leads, 73% and 88%. The sensitivities and specificities of the MLL lead for diagnosing AF were better than those for the 12-LEG. For diagnosing AF, the sensitivity and specificity of the MLL lead are better than those of the 12-LEG and MLL + V5 lead.Q:
